How good are your new hires, as one score you can track over time?
Each indicator becomes a percentage: performance = average rating ÷ the top of your rating scale; ramp-up = hires who reached full productivity on time ÷ hires; retention = hires still employed ÷ hires; manager satisfaction = your survey score. The score is the simple average of the indicators you fill in.

Pick a few indicators you can measure for every new hire — usually performance rating, how quickly they reach full productivity, whether they are still with you after a set period, and how satisfied the hiring manager is. Turn each into a 0–100 score and average them. That average is your quality of hire.
Quality of hire = (indicator 1 + indicator 2 + … + indicator n) ÷ n, where each indicator is a percentage. For example (76% performance + 80% ramp-up + 90% retention + 85% manager satisfaction) ÷ 4 = 82.8. This calculator weights every indicator equally and skips any you leave empty.
Once a hiring cohort has been in the job long enough for the indicators to mean something — often 6 to 12 months after they start. Measure the same cohort the same way each time so you can compare intakes, sources and recruiters.
Many companies do, for example weighting performance more heavily. This calculator uses equal weights so the score is easy to explain. If you weight, write the weights down and keep them fixed, or scores from different periods won't be comparable.
