Clinical
- Provides comprehensive, evidence-based nutrition care to all residents served by Americare Home Health Services, including adult and pediatric populations, ensuring all patients receive an initial nutrition assessment within 72 hours of admission (or as per service policy), with clearly defined goals for intervention.
- Conducts detailed nutritional assessments—anthropometric, biochemical, clinical, and dietary—to identify nutrition-related problems, estimate nutrient requirements, and guide treatment planning for both adult and pediatric patients.
- Develops, implements, and reviews nutrition care plans for both adult and pediatric patients, addressing medical conditions, developmental needs, feeding challenges, and cultural or religious dietary considerations.
- Documents all assessments, interventions, and progress in the medical record accurately and in compliance with facility and DOH documentation standards.
- Collaborates closely with nursing and physicians to support screening, nutritional monitoring, and timely follow-up for all patients.
- Educates patients, families, caregivers, and sitters on diet modification, feeding techniques, and appropriate nutrition management for adults, children, and medically complex patients.
- Communicates effectively within the multidisciplinary team regarding patient progress, dietary modifications, allergies, and nutritional risk.
- Evaluates prescribed diets and make necessary recommendations or modifications based on medical condition, growth patterns, and tolerance.
- Evaluates appropriateness of enteral nutrition support, including energy, protein, and adequacy of fluid provisions, making necessary recommendations in a timely and professional manner.
- Develops and updates patient and caregiver education materials, menus, and resources specific to adult, pediatric, and homecare populations.
- Identifies and reports any signs of developmental delay, neglect, or safeguarding concerns following organizational policies and procedures.
- Maintains clear professional and clinical boundaries when working with both adult and pediatric populations, ensuring all interventions are developmentally appropriate, ethically sound, and aligned with family-centered and culturally sensitive care principles.
- Assists in reviewing and developing departmental policies, procedures, and nutrition protocols that support best practices across all care environments.
- Participates actively in quality improvement (QI), risk management, and accreditation readiness activities related to nutrition care delivery and patient safety.
- Supports the management of departmental and physical resources, including participation in procurement decisions for nutrition-related equipment and materials.
- Contributes to community and caregiver education initiatives, promoting awareness of nutrition’s role in recovery, chronic disease management, and pediatric growth and development.
- Maintains ongoing competency in adult and pediatric nutrition practice through structured professional development, evidence-based learning, and reflective practice.
- Refers to the Knowledge, Skills, and Attitude Framework and Competency Summary Record for Allied Health for the detailed list of dietetics-specific competencies required for safe and effective practice across all age groups.
- Renews and validates job-specific competencies every two years or as required, ensuring continued compliance with DOH and Americare Home Health standards while supporting continuous professional growth.
